In this Masterclass, Dr Philip McMillan explored the importance of immune phenotyping as a practical framework for understanding chronic illness, long COVID, post-viral syndromes, and post-pandemic inflammatory patterns. The central message was that before asking “What should I take?”, the more important question is: “Which immune system are we dealing with?” Many people with similar symptoms — fatigue, brain fog, crashes, gut sensitivity, palpitations, pain, recurrent infections — may actually have very different immune drivers underneath. The session began by revisiting the ROOT roadmap — Review your health, Observe your progress, Optimise your plan, and Target long-term wellness. Dr McMillan again emphasized the concept of the inflammatory explosion, where prior immune activation acts like embers, and spike protein exposure from infection or vaccination can act like petrol, amplifying the inflammatory response. The goal is not simply to suppress symptoms but to identify and calm the underlying immune “embers.” Questions Asked After the Presentation What is immune phenotyping? → It is a way of classifying people according to the dominant immune pattern driving their symptoms, rather than simply grouping them by diagnosis or symptom labels. How is this different from symptom-based phenotyping? → Symptom-based phenotyping looks at systems such as gut, cardiovascular, neurological, upper airway, or vagal patterns. Immune phenotyping looks at the immune mechanism underneath, such as macrophage activation, mast cell reactivity, or immune exhaustion. Can both types of phenotyping be used together? → Yes. A person may have, for example, a gut phenotype with mast cell dominance, or a neurological phenotype with mast cell-driven headaches and migraines. Why does immune phenotype matter? → Because the same treatment can help one person and worsen another if the underlying immune driver is different. Why do the same symptoms need different strategies? → Fatigue, crashes, brain fog, or gut symptoms can arise from different immune mechanisms. The timing, triggers, and recovery pattern help identify which phenotype is dominant. How does someone begin to identify their own immune pattern? → By reviewing their history carefully, especially the pattern that existed before the pandemic. Longstanding allergies, hay fever, asthma, food reactions, or atopy suggest mast cell dominance. Recurrent infections and slow recovery suggest immune exhaustion. What suggests a macrophage-dominant pattern? → Flu-like heaviness, deep fatigue, aching, headaches, chest symptoms, post-exertional flares, and feeling poisoned, especially after exertion or infection. What suggests a mast cell-dominant pattern? → Rapid reactions to foods, smells, heat, supplements, alcohol, histamine-rich foods, fragrances, or medications, often with hives, itching, sinus symptoms, palpitations, flushing, or gut sensitivity. What suggests immune exhaustion? → Recurrent infections, prolonged recovery from viral illnesses, low stamina, poor stress tolerance, poor mucosal control, and feeling depleted. Can someone have more than one phenotype? → Yes. Many people have mixed patterns, especially after COVID, where macrophage activation, mast cell reactivity, and immune exhaustion can overlap. In a mixed phenotype, what should be treated first? → The dominant or earliest pattern should usually be addressed first, often identified from the person’s pre-pandemic history. Why can low-dose naltrexone help one person but not another? → Because it may be used at the wrong time or in the wrong immune context. The response depends on the person’s phenotype and treatment sequence. Why does Dr McMillan often stop supplements initially? → Because too many supplements can create noise, trigger reactions, and obscure the real pattern. Simplifying first helps identify what is actually driving symptoms. Where do omega-3 supplements fit into this discussion? → Omega-3s may support immune stabilisation, especially where deficiency or imbalance exists. Dr McMillan highlighted the value of measuring omega-3, omega-6, and omega-9 balance rather than guessing. Can dry skin suggest omega-3 deficiency? → It may. Dr McMillan noted that dry skin can sometimes improve when omega-3 status is corrected, although this depends on whether deficiency is actually present. What is the main takeaway from the session? → The first question should not be “What supplement should I take?” but “What is my immune pattern, what is the first target, and what sequence should I follow?”
